🏥 CBD 7-Day Medical Centre – Clinic Closure Notice

 

We regret to inform you that CBD 7-Day Medical Centre is now permanently closed.

 

Following the retirement of Dr. John Skala and after decades of dedicated service, the clinic has officially ceased operations.

 

We understand this change may be difficult, and we are here to assist you during the transition.

 

📁 Medical Records – Support Available Until 30th September 2025

 

We will continue supporting our patients with the secure transfer of medical records until 30th September 2025.

 

You have two options:

 

1. If you have already chosen a new GP:
   Please provide a Transfer of Medical Records Consent Form from your new clinic, including:

• Your full name
• Date of birth
• Name and address of your new GP clinic

 

 

Once we receive this, we will proceed with the secure transfer of your medical records to your new GP.

 

1. If you need help finding a new GP:
   Please contact us. We are happy to assist you in locating a suitable GP near you and will help facilitate the transfer once your new GP is confirmed and consent is received.

Please note: Medical records can only be transferred to another GP or medical provider with written consent. For privacy reasons, records cannot be released directly to patients.
